The position is responsible for creating clear, accurate, and compliant admission narratives that establish hospice eligibility for patients with terminal illnesses. Key Responsibilities: Write detailed and accurate admission narratives for patients with terminal illnesses, ensuring clarity and completeness Collaborate with healthcare teams to gather relevant patient information and clinical details Maintain confidentiality and adhere to all privacy regulations and institutional policies Review and update narratives as needed to reflect ongoing patient status and care plans Ensure all documentation meets organizational standards and regulatory requirements Review patient clinical documentation, including history, diagnoses, and recent clinical notes. Write clear, concise, and compliant admission narratives that establish hospice eligibility based on terminal prognosis. Collaborate with hospice nurses, intake coordinators, and other team members to clarify clinical details as needed. Ensure all narratives meet regulatory standards and organizational policies.
