You will work with an interdisciplinary team to deliver patient-and-family-centered care for individuals with serious life-limiting illness, both in the home and inpatient settings. Responsibilities: Provide consultative and direct patient care in palliative and hospice settings (home, skilled nursing facility, inpatient) Lead goals-of-care discussions with patients, families and the care team Manage complex symptom burden (e.g. pain, dyspnea, nausea) and coodinate care plans aligned with patient values. Collaborate with physicians, advanced practice providers (APPs), nursing, social work, spiritual care and case managers. Participate in quality improvement initiatives, program development, and education of medical staff.
