Your Day-to-Day: Conduct in-depth patient assessments, diving deep into physical, emotional, and holistic needs. Build tailored care plans that prioritize the voice and wishes of our patients and their families. Be the crucial link, communicating vital updates between physicians, patients, and caregivers. Experience: At least 3 years as a Registered Nurse, with 1 year in Hospice Care. Licensure: Registered Nurse in Pennsylvania & valid CPR certification.
