Hospice Registered Nurse Case Manager Salary: $80-89k/yr BOE Schedule: Full time | M-F| 8:00AM-5:00PM +(on-call rotation) Location: Kettering, Ohio, and surrounding areas Requirements of the RNCM role: * Licensed Registered Nurse * BLS Certified * Valid Drivers License and Auto insurance Coverage * Reliable transportation * A minimum of 1 year of Clinical experience * Hospice experience preferred About the RNCM role: * Coordinate hospice care by working with other team members as well as with the patient's family members to ensure needs are met. * Evaluate the effectiveness of hospice services to the patient and family on an ongoing basis. * Perform evaluations for hospice care along with informational visits as needed. * Consult with the attending physician and hospice medical director concerning changes in patient condition requiring order changes and/or changes in the plan of care. * Submit complete, accurate, and relevant clinical notes promptly. * Participate in IDG and case conferences * Performs on-call duties per policy and need * Conducts an initial and ongoing comprehensive assessment of the patients * Develop and implement the initial plan of care and work with other members of the IDG in completing and maintaining the plan of care. * Supervise and evaluate the care given by the hospice Aide at a minimum of once every 14 days or as needed. * Provide care plan to Hospice Aide and LPN as part of the IDG team. * Perform death visits per company policy * Participate in patient's discharge planning process Why work with Care 360 Hospice: * Medical, dental, and vision health Insurance * Paid Time Off * 401(k) Matching * Life Insurance * Growth opportunities NEXT STEPS We appreciate your interest in our Hospice Registered Nurse Case Manager.
