Atlantic Visiting Nurse, part of Atlantic Health System, is seeking a part-time hospice Registered Nurse to work 9 AM to 7 PM in the Morris County area, including weekends and on-call as required by department. In this position you will work with adult and geriatric patients of all backgrounds who require hospice care at one of our hospital sites, sometimes in their homes as needed. You are compassionate, thoughtful, and comfortable working with elderly and terminally ill patients. You will be responsible for managing a heavy caseload (4 patients a day); time management skills are essential. However, it is important to provide each patient with the time and attention necessary to meet his or her needs. Mileage reimbursement ($.70) included. Internal referral bonus: $4,000 Reports to: Assistant Manager Duties & Responsibilities: Performs Patient assessments. Accurately assesses patient’s appropriateness for hospice care, learning needs and progress toward goals Develops the plan of care for the patient and family Ensures appropriate utilization of services Maintains visit productivity standards Demonstrates knowledge of insurance reimbursement Completes discharge assessments Performs laptop transfer of data twice daily Completes admission assessment and transfers within 24 hours Completes re-visit documentation and transfers Uses age appropriate teaching methods for patients and families Evaluates teaching plan and revises as necessary Documents patient education tools used Assumes responsibility for Case Management. Develops/implements/evaluates plan of care based on evidence-based knowledge Revises care plan as indicated Prioritizes care needs Demonstrates knowledge of hospice regulations and insurance reimbursement guidelines Shows evidence of interdisciplinary communication Performs HHA supervisions per standards Ensures appropriate utilization of services Follows scheduling process Communicates progress toward goals with IDT Attends and participates in IDT meetings every 2 weeks Evaluates the plan of care and revises as needed Maintains safe environment for patients and co-workers Follows infection control measures and reports infections Demonstrates knowledge of pharmacology and safe medication management Routinely reconciles medications Maintains current medication orders Attends monthly staff meeting Participates in PI meetings and activities/committees as requested. Accepts other assignments as requested Assumes responsibility for case management. Prioritizes schedule to accommodate patient's needs Documents case management activities. Demonstrates knowledge of pharmacology and safe medication management Routinely reconciles medications. Maintain current medication orders. Participates in Performance Improvement Program Reviews health records and gathers data to support PI activities Attends UR meetings as requested Provides orientation/ mentorship to new employees Cross trains to learn new skills and demonstrates a willingness to teach other Assists with growing agency business Provides orientation to new employees Accepts other assignments as requested Cross trains to learn new skills Qualifications for Internal Candidates Required: 1. Graduate of an accredited Nursing Program- Associates Degree. 2. Current state licensure as a Registered Nurse. 3. BLS certification. Preferred: 1. Bachelor’s degree in nursing. a. All ASN must complete their BSN studies within five years of their start date. 2. Master’s degree in nursing or related field. 3. 1+ year of experience in Nursing or relevant area. 4. ACLS certification.
Responsibilities
The hospice Registered Nurse will perform patient assessments, develop and implement care plans, and manage a caseload of patients. They will ensure appropriate utilization of services and maintain documentation as required.
