Description The Hospice Registered Nurse (RN) provides compassionate, patient-centered care for individuals with terminal illnesses and their families. As a key member of the hospice team, you will deliver quality end-of-life care in a supportive and empathetic manner, focusing on comfort, symptom management, and emotional support. You will work collaboratively with physicians, social workers, chaplains, and other healthcare professionals to ensure a dignified and peaceful transition for patients. Schedule: PRN, Weekends, On-Call, Monday - Friday, 8:30 AM - 5:00 PM, possibility to transition into Full-Time Primary Responsibilities and Duties: Conduct thorough assessments of patients; physical, emotional, and spiritual needs. Develop and implement individualized care plans in collaboration with the patient, family, and other hospice staff. Administer medications, manage pain and symptom control, and perform medical procedures as needed. Monitor and document patient progress, changes in condition, and response to treatments. Educate patients and their families on disease progression, care options, and symptom management. Collaborate with physicians, therapists, and other healthcare professionals to coordinate and optimize patient care. Offer emotional and psychological support to patients and families, addressing their concerns and facilitating discussions about end-of life care. Coordinate with other members of the hospice team to ensure comprehensive and continuous care. Advocate for patients’ preferences and needs, respecting their values and wishes. Maintain accurate and up-to-date patient records and documentation in compliance with regulatory and organizational standards. Participate in on-call rotation as required to provide care and support after hours. Perform all other duties assigned. Requirements Job Qualifications: Valid license as a Registered Nurse (RN) in the state where work is performed. Minimum 1 year of nursing experience in hospice, palliative care, or related fields. Strong clinical skills and knowledge of nursing principles, procedures, and best practices in long-term care. Excellent communication and interpersonal skills, with the ability to interact effectively with residents, families, caregivers, and healthcare professionals. Demonstrated ability to prioritize tasks, manage multiple responsibilities, and adapt to changing priorities in a fast-paced environment. Understanding of regulatory requirements in hospice settings and nursing practices. Ability to build rapport with patients through a compassionate and caring demeanor. Physical Requirements: Ability to lift and transfer patients, with or without assistance, up to 50lbs. Capability to perform physical tasks such as bending, standing, walking, and climbing stairs for extended periods. Strength to handle medical equipment and assist patients with mobility and physical activities. Visual acuity and dexterity for performing tasks such as reading medical records, administering injections, and handling delicate medical instruments. Ability to respond swiftly and efficiently to emergency situations. Capacity to work in various home environments which may include navigating stairs, tight spaces, or uneven surfaces. Maintain regular attendance and timeliness as set forth within the work schedule designated by the department supervisor/manager. The job requires accepting criticism and dealing calmly and effectively with high-stress situations. The ability to use your abdominal and lower back muscles to support part of the body repeatedly or continuously over time without "giving out" or fatiguing. The ability to keep your hand and arm steady while moving your arm or while holding your arm and hand in one position.
