Description Position: Hospice RN Location: Benton Franklin County Type: Full Time 40 Anticipated Schedule: 4 10-hour shifts per week. VALUES: Accessibility, Integrity, Achievement, Supportive Environment Position Overview: As an In-Home Hospice Registered Nurse (RN) with Tri-Cities Chaplaincy, you will play a vital role in delivering high-quality hospice care in the comfort of patients' homes. Your responsibilities encompass providing skilled nursing care, pain management, and emotional support to patients facing end-of-life challenges. You will work collaboratively with a compassionate team of healthcare professionals to ensure patients receive the best possible care and comfort during their journey. Requirements Minimum Qualifications: 1. Associate Degree in Nursing. 2. Unrestricted Registered Nurse Licensure in Washington State 3. Must possess and maintain a valid driver’s license, as driving is an essential function of this role. Must meet insurance eligibility requirements. 4. Must have reliable transportation and the ability to independently drive within Tri-Cities Chaplaincy’s service areas to provide in-home patient and family services and support in clients' homes, facilities, and community locations 5. Moderate computer skills. 6. Ability to work independently and as part of a team. 7. Available to work on-call a minimum of two-holiday shifts per year. 8. This position is designated as safety-sensitive and will require passing pre-employment drug testing, which includes cannabis testing in WA State. Preferred Qualifications: 1. Bachelor’s Degree in Nursing. 2. Bilingual in Spanish. AGENCY REQUIREMENTS 1. Mission, Vision, and Values: Demonstrate support and adherence to the agency’s Mission, Vision, and Values. 2. Community Outreach: Participate in education and outreach activities to promote agency programs, including hospice, palliative care, advanced planning, and bereavement services. 3. Policy Compliance: Maintain knowledge of and comply with agency policies, procedures, and practices. 4. Confidentiality: Protect patient and agency information per HIPAA guidelines. Only access information on a need-to-know basis. 5. Health Requirements: Satisfy state and agency health requirements by providing proof of immunization, declinations, or approved accommodations as applicable. 6. Screenings: Pass criminal background and drug screenings. 7. Physical Demands: Meet the physical and environmental requirements of the position, with or without reasonable accommodation.(see attached physical requirements) 8. Training and Orientation: Complete agency and departmental orientation and required training programs. 9. Continuing Education: Complete ongoing education, policy reviews, and training by designated deadlines. 10. Licensure and Certification: Maintain current licensure(s) and certifications(s) required for the role. 11. Language Proficiency: Read, write, speak, and understand English fluently. 12. Work Availability: Be available to work as assigned or scheduled. RESPONSIBILITIES AND ESSENTIAL FUNCTIONS 1. Assume primary responsibility for a patient/family caseload that includes the assessing, planning, implementing, and evaluating phases of the nursing process. 2. Provide skilled nursing assessment, planning, and care to maximize the comfort and health needs identified by the patient/family unit. 3. Coordinates services to provide preventive and supportive care to the terminally ill and their families in accordance with approved care plans and Hospice policies and procedures. 4. Directs the work of the Licensed Professional Nurse and Certified Nursing Assistant as assigned. 5. Ensure compliance with IDG meetings at a minimum every 15 days and as needed. 6. Patient care documentation entered into EMR within 24 hours. 7. Patient care plan entered into EMR prior to Nursing Assistant assignment. 8. Regularly attend scheduled staff meetings and in-services. 9. Update facility staff on changes to care plans and maintain regular communication with administration staff at facilities of assigned patients. 10. Utilize and order resources such as medical supplies, medications, and DME appropriately. 11. Initiate communication with attending physician(s), other hospice staff members, and other agencies as needed to coordinate optimal care and use of resources for the patient/family. 12. Maintain regular communication with the Clinical Nursing Supervisor to review caseload. 13. Maintain regular communication with the attending physician concerning patient/family care needs and response to interventions. 14. Obtain data on physical, psychological, social, and spiritual factors that may influence patient/family health status and incorporate that data into the plan of care. 15. Seek input from other team members regarding the patient’s plan of care to obtain additional knowledge and support. 16. Maintain up-to-date patient records so that problems, plans, actions, and goals are accurately and clearly stated, and changes are reflected as they occur. 17. Instruct primary caregivers, volunteers, and employed caregivers to provide care as indicated. 18. Meet regularly with the interdisciplinary team members to review problems or unique issues from caseloads, share professional support, and exchange feedback aimed toward enhancing professional growth. Compensation Hiring Range:$37.67/hour - $57.22/hour Position Pay Range:$37.67/hour - $71.57/hour Note regarding the listed pay range: Hiring Range: The starting pay range offered to new hires, depending on experience and qualifications.
