OVERVIEW

We are seeking a dedicated and experienced Registered Nurse Case Manager to join our healthcare team. The ideal candidate will have a strong background in various nursing specialties and will be responsible for overseeing patient care from admission through discharge. This role requires excellent communication skills, critical thinking, and the ability to work collaboratively with healthcare professionals to ensure optimal patient outcomes.
Must have at least 1 year of hospice experience within the last 3 years
Oasis experience is required
$2,000-$2,150/Wk plus .42 a mile for patient related travel
Monday-Friday 8-5
1 on call weekday shift a week

EXPERIENCE

Candidates should possess the following qualifications:

  • Critical care experience in settings such as ICU, emergency room, or Level I/II trauma centers is highly desirable.
  • Familiarity with various nursing specialties including geriatrics, pediatrics, infectious disease care, behavioral health, medical-surgical, hospital medicine, and outpatient clinic experience.
  • Proven ability in case management practices including intake assessments, discharge planning, diagnostic evaluation, and utilization management.
  • Strong knowledge of physiology, medical terminology, vital signs monitoring, and patient service protocols.
  • Previous experience in acute care settings is essential; post-acute care experience is a plus. Responsibilities
  • Conduct comprehensive patient assessments to develop individualized care plans.
  • Coordinate and manage patient care across multiple settings, including inpatient, outpatient, and post-acute care.
  • Collaborate with interdisciplinary teams to facilitate effective discharge planning and transitions of care.
  • Monitor patient progress and adjust care plans as necessary based on ongoing evaluations.
  • Ensure compliance with HIPAA regulations and maintain accurate medical records.
  • Provide education and support to patients and their families regarding treatment options, resources, and care management strategies.
  • Utilize critical thinking skills to evaluate diagnostic information and make informed decisions regarding patient care.
  • Advocate for patients’ needs within the healthcare system while promoting effective utilization management.
