Position Details: Full-time Hospice RN Case Manager position Schedule: Monday thru Friday, 8a-5p Johnson County, Marion County and Surrounding Communities Benefits: Competitive salary Mileage reimbursement (currently 50 cents per mile) Excellent PTO program (up to 136 hours in first year) Every Friday is payday! Health, dental, vision 401k match program Great team environment! Job Functions: Performs physical and psychosocial assessment on all hospice patients. Ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management. Responds to the physical, mental, social and emotional needs of terminally ill patients and their families. Assesses patient’s condition, initiates plan of care, re-evaluates and updates as necessary. Obtains all necessary physician orders and collaborates with physician as patient’s needs dictate. Initiates case conferences to discuss multidisciplinary team responsibilities, pain management, bereavement counseling, etc. Requirements Experience: 3 years Hospice experience 5 years of RN experience Credentials: Licensed Registered Nurse in State of Indiana Current driver's license Must have reliable transportation and insurance

Responsibilities

The Hospice RN Case Manager performs physical and psychosocial assessments on hospice patients and initiates care plans based on their needs. They also collaborate with physicians and lead case conferences to ensure comprehensive patient care.
