Our mission to become the 'Employer of Choice' has led up to wining a 'Top Workplace' Award by the Indy Star for the past 5 years, and we are not finished yet! \n Job Description Full-time Hospice RN Case Manager position Schedule: Monday thru Friday, 8a-5p Available territory openings: Case Manager typically assigned to cover 1-2 counties near the Office. Performs physical and psychosocial assessment on all hospice patients. Ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management. Responds to the physical, mental, social and emotional needs of terminally ill patients and their families. Assesses patient’s condition, initiates plan of care, re-evaluates and updates as necessary. Obtains all necessary physician orders and collaborates with physician as patient’s needs dictate. Initiates case conferences to discuss multidisciplinary team responsibilities, pain management, bereavement counseling, etc Benefits Competitive salary Mileage reimbursement Excellent PTO program (up to 136 hours in first year) Every Friday is payday! Responsibilities

The role involves performing physical and psychosocial assessments on all hospice patients, managing pain through assessment and education, and responding to the physical, mental, social, and emotional needs of terminally ill patients and their families. The RN will also initiate and re-evaluate the plan of care, obtain physician orders, and collaborate with the physician as needed.
