Job Title: RN Job Overview: As an RN of Luminos Hospice you will work with the Clinical Director to lead a care team including LPN(s), LVN(s), Aides, MSW and Chaplain to provide best in class hospice care. Responsibilities Coordinates and oversees all aspects of patients' home care. Collaborates with patient care teams to develop and implement care plans. Educates families on patient condition, needs and care. Performs head to toe physical assessments. Checks vital signs and manages medications. Administers and maintains catheters. Complete documentation timely and accurately. Supervises HHAs. Regularly communicates patient progress to the clinical manager, physician and care team. Other duties as assigned. Requirements Hospice Registered Nurse / RN Case Manager Requirements RN license in the state of MO Current driver’s license, reliable transportation, valid auto insurance Current CPR and BCLS certification Excellent verbal and written communication skills Minimum two years experience as an RN, hospice experience preferred Job Type: Full-time Benefits: Medical, dental, and vision insurance HSA and FSA plans 160 hours of PTO annually and 9 paid holidays Life insurance paid by company Disability insurance Mileage reimbursement of 50 cents/mile

Responsibilities
