Description
Overview Hospice Social Worker provides a wide variety of direct and indirect social work services to patients/caregivers/families according to the individualized plan of care as prescribed by the physician. The Hospice Social Worker actively participates in coordination of all aspects of the patient’s care. The Social Worker participates in ongoing interdisciplinary comprehensive assessments, develops and evaluates the Plan of Care, contributes to patient and family counseling while also participating in the education of the QUAPI program and hospice sponsored in-service training. Schedule: Part-time variable days with requirement to assist with social work coverage for current staff and to carry a caseload Location: Falls Church, VA - Homecare COVID-19 vaccine required to be completed upon start Responsibilities The Hospice Social Worker is an active member of the Capital Caring Health Interdisciplinary and participates in ongoing interdisciplinary comprehensive assessments. Qualifications Experience Requirements Two years of master’s level experience in an interdisciplinary health care setting is desired. Education Requirements Must have a Master's degree in Social Work from a school accredited by The Council on Social Work Education. Licensing Requirements Must be licensed or license eligible in compliance with jurisdiction of practic (LCSW, LICSW). Driver's License Must have a driver's license in good standing and access to a vehicle during working hours.
