Description

Hospice Social Worker Salary: $65-$78k/yr (BOE) Schedule: M-F| 8:00AM-5:00PM +(on-call rotation) Location: Hilliard, Ohio and surrounding area Requirements of the Hospice LSW role: * Licensed Social Worker in state of practice (LSW) * Master's Degree in social work preferred (MSW) * One year of social work experience in a healthcare setting is preferred * Current Driver's License, vehicle insurance, and access to dependable transportation. About the Hospice LSW role: * Provide medical social services to the patient and/or family according to the Plan of Care. * Conduct a psychosocial assessment as part of the comprehensive assessment within 5 days of admission to the hospice program, including emotional, social, financial, and environmental resources, and identify appropriate psychosocial problems, interventions, and goals for the patient's care. * Participate in IDG meetings and activities and the development of the patient plan of care. * Observe, record, and report changes in the patient's emotional and social factors that are being affected by the patient's illness and his/her need for care and response to treatment. * Maintain and submit written clinical records as deemed by Care 360 Hospice, including the initial evaluation, the care plan, and daily notes. * Confirm weekly, the scheduling of visits with the patient's case managers to facilitate coordination of other staff visits. * Assess the patient's and family member's ability to cope with the patient's passing. * Act as a consultant to the members of the health team, assisting them in understanding the social, emotional, and environmental factors related to the patient's health problems. * All other duties as assigned Why work with Care 360 Hospice: * Medical, dental, and vision health Insurance * Paid Time Off * 401(k) Matching * Life Insurance * Growth opportunities NEXT STEPS We appreciate your interest in our Hospice Social Work position.
