We are seeking a dedicated and empathetic Hospice Social Worker (MSW preferred) to join our interdisciplinary care team. This role offers the opportunity to make a profound difference by providing emotional, social, and practical support to patients and families facing serious illness. Key Responsibilities * Provide psychosocial support, counseling, and education to hospice patients and their loved ones. * Conduct assessments to identify emotional, social, and financial needs. * Collaborate with nurses, aides, chaplains, and physicians as part of an interdisciplinary care team. * Assist families with advance care planning, community resources, and bereavement support. * Advocate for patient rights, dignity, and preferences. * Maintain accurate, timely documentation in accordance with agency and regulatory standards. Qualifications * Licensed Social Worker (LSW/LCSW/LMSW or equivalent state licensure). * Experience in hospice or healthcare setting preferred. * Exceptional communication, counseling, and advocacy skills. * Compassion, flexibility, and the ability to support patients and families through emotional transitions. * Valid driver’s license and reliable transportation required.
