This role involves collaborating with the Interdisciplinary Group/Team (IDG/T) to coordinate care and ensure that patients receive holistic, compassionate support tailored to their needs. You will also participate in an on-call rotation as required by the needs of the hospice. Key Responsibilities: Conduct psychosocial assessments to evaluate the emotional, social, and environmental factors impacting the patient and family. Provide emotional support and counseling to patients and families coping with end-of-life issues. Coordinate care with the IDG/T to ensure all aspects of patient care are addressed. Assist families in understanding and managing grief and loss. Help families access community resources and services. Maintain documentation of social work services in accordance with hospice policies and regulatory requirements. Participate in on-call rotation as required. Requirements Education: Master’s degree in social work (MSW) from an accredited school of social work Licensure: Current state licensure as a social worker required Current driver’s license and reliable transportation Experience: Minimum two (2) years of experience in a healthcare setting, preferably in hospice or palliative care Strong working knowledge of community resources and support services Skills: Excellent interpersonal and communication skills Ability to use computers for documentation and communication purposes Compassionate, empathetic, and professional demeanor
