We’re hiring a Regional Triage Nurse (RN) – a skilled and compassionate professional who provides telephonic triage, clinical oversight, and coordination of after-hours care across multiple hospice sites. This role is essential to ensuring timely, high-quality care for patients and families during critical moments. What You’ll Be Doing Oversee and assign on-call/after-hours RN runners for supported hospice sites Provide telephonic assessments and implement appropriate interventions based on patient’s terminal diagnosis and reported symptoms Collaborate with attending physicians, Medical Directors, and interdisciplinary hospice teams Utilize EMR systems (HCHB workflow) as assigned Act as liaison with patients, families, and healthcare professionals while maintaining confidentiality and dignity Qualifications Active RN licenses in good standing in both California and Texas Minimum of 1 year RN experience, including at least 1 year in end-of-life care Certification as a Hospice and Palliative Nurse (CHPNA) preferred but not required Must clear criminal background check, physical, and drug screening Valid driver’s license, good driving record, and reliable transportation required Willingness to travel to patient locations as needed Ability to work outside regular hours depending on business needs Why Choose Silverado?
