This is an on-site position located at Newton-Wellesley Hospital. Job Responsibilities: The position of Hospital-Based Hospice Nurse Liaison (HBHNL) is to provide information about hospice services to patients, families, and hospital-based staff, to conduct clinical assessments, coordinate hospital discharge plans, and maintain professional relationships within hospitals, with a focus on the assigned hospital. Schedule and Weekly Hours: Monday through Friday from 9 am to 5 pm (40 hours) Education, Skills and Experience: Licensed by the Commonwealth of MA as a Registered Nurse. At least two years experience preferred in community health, oncology, hospice, or palliative care. Excellent communication and interpersonal skills Ability to work collaboratively with hospital staff, Good Shepherd colleagues, patients, and their families Highly skilled at providing patient assessments and evaluations. Excellent problem-solving skills Competency with accepted standards of hospice nursing practice. Individual pay is determined through a variety of factors, including experience, education, certifications, and skills and can exceed the starting range.
