Hudson Wide Health Care has an exciting opportunity for an experienced Hospital Liasion/Marketer for SNF in Dutchess County area

REQUIREMENTS:

• Prior experience as post-acute and long-term care Admissions Director, Admissions Coordinator or Admissions Screener.
• Ability to work independently and with Admission Directors and the interdiscplinary team
• Experience with electronic medical records including EPIC, Allscripts and Navi.
• Conducting sales and marketing duties at area hospitals and community events.
• The position is a hybrid of work in the community and in-house in our Dutchess County facilities.
• Meeting and maintianing relationships with patients and their families during the admissions process
• Previous experience with long term care is required.

• Coordinating the referral and approval process in a timely manner.
• Establishing & maintaining relationships with local hospitals, physicians , assisted living , adult care centers & community referral sources.
• Complete the admission process from screeing to ensuring the arrival of the resident, to include all insurance and clinical documentation.
• Obtain insurance authorization for admission
• Independently conduce facility presentations and on-site tours.
