Iowa City Hospice is seeking a compassionate and dedicated Hospice Hospital Liaison Nurse to join our team. The ideal candidate will serve as a vital link between our hospice services and hospitals, ensuring seamless communication and coordination of care for patients transitioning to hospice.

Position Summary: You will work closely with patients, families, and a dedicated interdisciplinary team to provide comfort, support, and expert clinical care. The ideal candidate will enjoy an autonomous work setting and have a passion to provide compassionate palliative end of life care.
Key Responsibilities:- Collaborate with hospital staff to identify patients who may benefit from hospice services.- Conduct assessments and provide education to patients and families regarding hospice options.- Facilitate smooth transitions of care from hospital to hospice settings.- Maintain accurate documentation and communication with all stakeholders involved in patient care.- Participate in interdisciplinary team meetings to discuss patient care plans and updates.
Skills and Qualifications:- Registered Nurse (RN) with a valid nursing license.- Experience in hospice, palliative care, or acute care settings preferred.- Strong communication and interpersonal skills.- Ability to work collaboratively in a team-oriented environment.- Compassionate approach to patient care and family support.- Excellent organizational and time management skills.

KNOWLEDGE/SKILLS:

  • Overall, the role requires strong interpersonal skills, negotiation abilities, and a deep understanding of healthcare operations, regulations and cultural differences.
  • Demonstrates competence in professional decision-making skills within the parameters of the Nurse Practice Act, including the ability to make appropriate assessments, problem-solve, prioritize, complete Plans of Care and evaluate their effectiveness.
  • Demonstrates initiative in recognizing unmet needs and assuring a timely and appropriate response, skills and knowledge in providing education and treatment of acute and chronic symptom management.
  • Maintains confidentiality in all matters, including employee information, client information and agency matters.
  • Knowledge of computer operating systems, data sourcing, project management with outcomes analysis proficiency required.
  • Demonstrates organizational skills necessary to manage workload and productivity expectations while working independently and managing several projects simultaneously.
  • Exhibits professionalism in all workday interactions, follows through on commitments and maintains composure under stress.
  • Recognizing new referral opportunities and ensuring that the organization responds appropriately to them.
