Responsibilities

This role is a contracted 12-month, full-time position. You will be based onsite at the Hollywood Private Hospital in Nedlands, with onsite paid parking.
Unlike community pharmacy, where the focus is often on volume and retail, Hospital Pharmacy offers a dynamic, fast-paced environment where precision and collaboration are key, and your decisions will have a direct impact on the treatment outcomes of patients.

Responsibilities

  • Conduct medication chart reviews and medication reconciliation.
  • Dispense and supply medication and pharmacy products.
  • Coordinate patient medication requirements for inpatient stay, day infusions and discharge.
  • Develop and maintain professional relationships with key stakeholders to ensure excellent customer service to clients and patients.
  • Support patients, carers, hospital staff and associated employees.
  • Educate and advise staff and customers on health promotion, disease prevention, drug selection and the proper use of medicines.
