Job DetailsJob Location: Christopher House - Austin, TX 78702Position Type: 40 HRS - 1 FTEJob Shift: M-F 8A-5PJOB TITLE: Hospital Registered Nurse DEPARTMENT: Christopher House POSITION SUMMARY The Hospital Registered Nurse will respond to GIP (general inpatient) visits for all hospitalized patients in the assigned facility. This position will cultivate relationships with the assigned facility, as well as educate the hospital professional community on hospice and end-of-life. This position will be asked to perform transitional care from the hospitals to Christopher House. WORKING RELATIONSHIPS REPORTS TO: Team Leader SUPERVISES: None INTERPERSONAL RELATIONSHIPS: All comments, actions and behaviors have a direct effect on the public’s perception of Hospice Austin. Interactions with patients, family members, physicians, referral sources, visitors, co-workers, etc. must be in a manner that is courteous, respectful, cooperative and professional. This behavior should promote an atmosphere of teamwork and positive relations. ESSENTIAL FUNCTIONS Performs transition visits for patients GIP in the hospital on hospice services (40%) Visit patients in hospital, perform GIP assessment, i.e. vitals signs, pain assessment, etc Determine appropriateness for GIP. Complete documentation for transfer to Hospice Austin’s Christopher House Teaches patient and caregivers regarding inpatient care- disease process, death and dying, comfort measures, medication use and side effect, etc. Initiates Plan of Care, including coordination of personal care, DME and obtaining medications and medical supplies. Documents in a timely manner. Cultivates and maintains relationships with and educates the hospital professional community. (30%) Cultivates, develops, and enhances care relationships with assigned hospitals. Revised: November 2024 FLSA Status: Exempt Develop a communication system with the Clinical team at the hospital to identify delivery failures and relay them to the Admissions team. Attending care conferences, and other applicable meetings within the assigned hospitals. Collaborates with Community Educators to develop formal and informal education opportunities in the hospital settings, with hospital staff, physicians, hospital social workers/case managers/discharge planners. Collaborates with Community Educators to develop and implement hospice educational plans within the hospital's professional community. Performs admissions process and coordinates care on an as-needed basis for admissions to HACH. (30%) Performs Initial Admission Assessments, i.e. vital signs, pain assessment, clinical summary, etc. Completes documentation for admission to services if appropriate. Qualifications Current RN license in the State of Texas. CPR certification. 1 year of nursing experience. Valid Texas driver’s license, auto liability insurance and safe driving record. Knowledge of hospice federal and state regulations preferred. Strong computer skills including knowledge of and experience with Microsoft Office products and experience with electronic medical records (EMR). Strong interpersonal and communication skills. Able to build and maintain professional relationships with hospitals and medical providers. Compassionate yet informative bedside manner. PREFERRED SKILLS: 1 year of previous hospice nursing experience.
