BEFORE YOU KEEP READING

Please do not see everything in this job advert as a “Must Have”, but rather a guiding list of what we are looking for. We know no candidate will be the perfect match for all we have mentioned in this advert, so do not be afraid to apply if you feel you are close to the brief but not “Spot On”. For example, some of our wonderful Carer Wellbeing Workers come from a non-social care background and they do amazingly well!

Responsibilities
  • Triage: Oversee and organise the Hospital Team action list and manage referrals to the Hospital Service, liaising with the referrer if required.
  • Co-ordinate Appointments: Where required contact carers to arrange appointments for one-to-one support on the phone or on-site in one of the hospitals served, ensuring carers can access the service easily and in a timely manner.
  • Referrals: Manage new referrals into our Support at Home after Hospital service, liaising with the referrer if required. Add new carers to the database and call carers to arrange an appointment time for assessment.
  • Demonstrate Impact: Support the collection and maintenance of data for reporting. Showcase the difference the service makes in carers’ lives through direct conversation to gather carer stories, evaluations and feedback.
