Description THIS POSITION REQUIRES PRIOR EXPERIENCE IN ORDER TO BE QUALIFIED: Essential Duties: • Complete the full intake process of a bio-psych-social, assess needs, identify barriers, and establish initial mutually agreed upon goals. Coordinate effective and efficient utilization of social service agencies to achieve goals and remove barriers for guests. • Communicate with outside community service providers and landlords in an effort to keep guests coming from shelter to safe housing stable within their home. • In conjunction with the guest, collaborate with other human service agencies and landlords supporting guest's needs. • Work in close contact with other departments, continually seeking additional resources for guests and provide "success" stories and other positive guest progress information. • Perform administrative tasks, maintain accurate records, and manage relevant data timely and accurately. • Accurately document pertinent actions, conversations, and events on guest's record that facilitates progress toward goal attainment, collective pertinent data in the online database. • Conduct relevant assessments as needed with guests that may result in a referral to outside services including other social service agencies, behavioral health and medical providers. • Participate in the development and implementation of guest care policies and protocols to provide clear expectations of both team members and guests. • Participate in general operations of the ministry as a vital member of the team. • Establish effective relationships of trust with guests. • Actively participate in relevant, cross-departmental meetings, team meetings, multidisciplinary team meetings with other agencies, landlords and training sessions. Will participate in relevant guest accountability team meetings. • Provide "success" stories and other positive guest progress information to the Philanthropy Team. • Conduct Case Reviews and Multi-Disciplinary Team Meetings with other Case Managers and agencies as required. • Cultivate and maintain an active list of landlords who will collaborate. • Assist in education classes, groups and seminars conducted within and outside the agency. Other Duties: • Regular attendance and punctuality are the requirements of this position. • Conduct life skills classes and related meetings as needed. • Maintain healthy boundaries with guests and team members in accordance with LifePath's policy while serving high-need individuals in crisis. • Other duties as assigned. Requirements Education/Qualifications: • Bachelor's degree from an accredited college or university in related discipline or minimum two years of related experience• Must be willing to sign a statement of faith. • Previous human services experience desired. • Possess a valid Pennsylvania Driver's License. Other Factors to Consider: • Ability to stand and/or sit for long periods of time, reach, lift, bend, kneel, stoop, climb stairs, push, and pull items weighing 25 pounds or less; manual dexterity; auditory and visual skills; ability to follow written and oral instructions and procedures. • Excellent reading and writing skills; typing/computer usage; proficient interpersonal relations and communicative skills; ability to follow written and oral instructions and procedures. • Frequent close quarter contact with mission guests as they enter and exit the building. • Fast paced environment; moderate noise level. • This position will require local travel using your personal vehicle when conducting in-home case management sessions.
