General Description The full-time Case Manager will provide direct one-on-one case management, develop and monitor service plans, assist with skill building, make referrals and advocate for clients. Candidates will show knowledge of community providers/services, crisis management and non-traditional communication styles. Case managers are supported by training, supervision, clear guidelines, and a robust referral system. Essential Duties and Responsibilities Provides housing case management services to clients in each of the 8 apartments. Responsibilities include assessing client needs, making referrals to appropriate community resources, and conducting intake sessions, including one half-day per week on-site at DSS. Conducts regular weekly visits with homeless clients to assess needs and provide resources, follow-up support, assist clients with budgeting and financial goal setting. Create and update IPL’s for all clients. Assists program participants in assessing their financial situation, creating household budgets, and establishing financial goals. Provides transportation when necessary. Ensures all client interactions are thoroughly documented in the client intake database, maintaining accurate and up-to-date case notes. Attends CCSL staff meetings, and other required meetings Attends CCSL functions as appropriate Ensures that all CCSL clients are treated with dignity and respect, always upholding their confidentiality. Requirements Qualifications Education: Associate’s degree in psychology/human service/related field or High School Diploma and 3 years of relevant experience. Credentials: Possession of a valid NYS Driver’s license and a driver's record considered acceptable by agency and insurance carrier Experience: At least 3 years of experience in social services, case management, or a related field is preferred.
