Position Summary: Working under the guidelines of the Center of Excellence, the Housing Case Manager provides formal case management services to eligible opioid-dependent individuals to promote stability, recovery, and healthy long-term outcomes. This role focuses on service coordination, housing stabilization, advocacy, and connection to community resources to support clients across the continuum of care. Retirement Plan with matching opportunity Employee Assistance Program Essential Functions: The Housing Case Manager is responsible for a wide range of duties, including but not limited to the following: * Provide service coordination, including case management, care coordination, client advocacy, and structured planning to help clients achieve individualized goals. * Meet regularly with clients to implement housing plans, stabilize housing situations, and develop life plan goals. * Encourage progress toward goals related to budgeting, employability, family supports, counseling, and linkage to community resources. * Maintain relationships with landlords and community resources to identify and secure housing opportunities for clients. * Document service coordination activities throughout the continuum of care in a timely and accurate manner. * Assess Service Plans and, in consultation with clients, make appropriate updates to support progress toward treatment and recovery goals. * Arrange advocacy services related to housing, court involvement, family services, or other supports impacting client well-being. * Maintain an updated list of local agencies, key contacts, and required documentation to streamline housing and service access. * Maintain ongoing contact with clients and identified support systems to support adherence to treatment and recovery plans. * Provide transportation support as needed to reduce barriers to care. * Establish and maintain collaborative relationships with civic groups, agencies, professionals, governmental entities, and community partners to ensure appropriate referrals and identify service gaps. * Apply confidentiality regulations appropriately at all times. * Prepare and submit required reports, documentation, and paperwork in a timely manner. * Attend required meetings, core trainings, addiction-related trainings, and community outreach events as necessary. * Maintain required certifications and complete minimum annual training hours. Minimum Education / Certifications: * Bachelor’s degree preferred. * In lieu of a Bachelor’s degree, a minimum of ten (10) years of relevant experience is required and enrollment in a Bachelor’s degree program in Social Work, Case Management, Social Science, Psychology, Addictions, or a related field (at least part-time) within the first six months of employment. Minimum Work Experience: * Minimum of one (1) year of case management experience preferred. Other Requirements: * Reliable transportation to travel throughout the service area.
