Description Summary: A responsible individual, who possesses knowledge about substance abuse and mental health under professional supervision, is capable of directing and motivating clients toward abstinence and sobriety through specific avenues of treatment. Essential Duties & Responsibilities: Assist in providing each client access to necessary clinical, medical, social, educational, rehabilitative, vocational, and/or other services. Perform treatment planning related task including coordinating appointments and placements and following up with clients. Complete all required documentation related to treatment in accordance with established procedures including but not limited to treatment plans, contact notes, periodic assessments, court letters, gathering medical records, copying, faxing, scanning, collateral contact, obtaining releases of information and appointment scheduling. Collaborate with the clinical team to identify required services by assessing the overall needs of each client. Provide linkages to various community resources, and coordinate care with all service providers. Support client recovery by facilitating various groups in house/community as well as individual case management with clients. Provide education, support and consultation to individuals’ families and their support system which is directed exclusively to the well-being and benefit of the individual, if needed and after obtaining clients’ consent with a release of information. Participate in admission and discharge process of residents. Perform related duties as required including transportation of clients in an insured and reliable vehicle. Responsible for clients and the facility safety and security. Substance abuse screening with the appropriate follow up. May supervise resident meal preparation, clean up and daily chores. May supervise resident self-administration of medication Capable of responding appropriately to emergencies and incidents as they happen. Must be awake and alert during shift hours. *All agency staff are required to attend all mandatory department/agency meetings and trainings* Schedule: Tuesday - Saturday, 8:00 am - 4:30 pm Requirements Education &/Or Experience: High School Diploma plus must be working towards an Associate Degree. Two years’ experience working with substance abusers in a residential setting preferred. Ability to work with diverse population. A familiarity with Twelve Step Program. Working experience at least one year.
