Job Details Job Location: Oakland - Jefferson Office - Oakland, CA 94607 Position Type: Full Time Education Level: 4 Year Degree Salary Range: $34.50 - $37.86 Hourly Job Category: Nonprofit - Social Services Position Summary: The Housing Services Case Manager (HSCM) assists those who are at risk of becoming homeless and/or precariously housed to secure and maintain permanent, stable, affordable housing in Alameda County. The HSCM provides housing-related financial assistance and case management. This position will complete applications for various housing services programs and other public and private grants to assist with back rent, security deposits, and utilities to stabilize housing. This position will also utilize housing problem-solving skills as needed. This position will provide crisis intervention, case management, and referrals. Additionally, this position may do 60-day and 6 months follow ups to evaluate effectiveness of the services provided. Essential Responsibilities: Conduct eligibility screenings over the phone or in-person for various housing assistance programs, including rental, mortgage, utility payment, relocation, and move-in assistance. Learn Housing Services program guidelines and procedures; interpret and apply guidelines; organize and prioritize work; perform highly detailed work with accuracy; prepare applications and client documentation. Meet weekly with supervisor to go over cases. Facilitate and participate in weekly office hours and housing clinics as needed. Conduct needs assessments to determine co-occurring issues or underlying factors contributing to housing crisis or insecurity. Gather, records, and evaluate information in Salesforce or other client management systems. Assist clients to identify their own resources necessary to secure and sustain housing using housing problem-solving techniques where necessary. In collaboration with the client, develop a housing stabilization service plan. Complete applicable applications and submit them; Prepare and submit check requests; insure proper documentation and disbursement. Provide information and referrals and follow-up with clients as needed. Help link community members to eligible county services, Social Security Insurance, Social Security Disability Insurance, Food, Housing, Transportation, Primary Care, Mental Health, Educational and Employment Supports and Opportunities, and all other services that will help support the consumer’s housing stability. Maintain accurate client records, including database entries, organize client file. Submit monthly client success stories. Maintain confidentiality; exercise objectivity; explain aspects of program to clients; work as part of a team; develop and maintain cooperative working relationships with state and other program-related agencies. Represent agency and programs at community events. Attend housing trainings and webinars. Perform basic arithmetic calculations; and recognize and refer complex eligibility and case management problems to supervisor. Perform conflict resolution to disgruntled clients as appropriate; assist and address client issues. Assist with quarterly case record reviews ensuring that program case files are updated and corrected as needed. As needed, receive referrals from community partner agencies (churches, community centers, etc.) and work with them to assist their clients with financial support. Performs other duties or functions as assigned. Qualifications Bachelor’s Degree or equivalent work experience required; Three or more years providing social services to the homeless or similar high-risk population. Familiarity with housing issues preferred. Reliable transportation to conduct home visits and conduct community outreach as needed Fluent in Spanish highly preferred Lived experience with housing services highly preferred Experience and sensitivity to the needs and issues of a diverse population. Ability to establish and maintain trustful, effective working relationships with diverse clientele. Basic computer skills: Microsoft Word, Outlook, Excel, and basic Microsoft Office applications. Excellent listening skills to accurately understand and assess client needs. Strong judgment to make informed decisions regarding program eligibility. Ability to work in a fast-paced, demanding environment. Strong organizational skills to manage and schedule client appointments. Effective communication skills to connect clients with additional resources. Excellent oral and written communication skills. Ability to travel locally for meetings and client home visits as needed. Current valid California driver’s license and proof of insurance (mileage reimbursed).
