JOB SUMMARY Under the supervision of the Director of Housing and Outreach, the Housing Support Case Manager provides comprehensive case management services to single adults and or families living within Eva’s Shelters, transitional housing programs or Passaic County. Housing Support Case Manager assists Medicaid-eligible individuals with obtaining and maintaining safe, stable, and affordable housing while promoting independence, improving health outcomes, and reducing unnecessary hospitalizations, emergency room visits, and institutional placements. All qualifying individuals will be considered chronically homeless or facing housing insecurity with active Medicaid. Case managers will assist individuals with presumptive eligibility screenings, maintaining active benefits and MCO Authorizations. Case managers will be tasked with providing person-centered, trauma-informed services that address social determinants of health through comprehensive care coordination, housing stabilization, advocacy, and collaboration with healthcare providers and community partners. Case managers will draft and monitor service plans for each client and or family. This role is specifically attached to the Medicaid housing support initiate and funded through Medicaid billing. This role will require field work in combination with work across multiple programs within Eva’s village. JOB DUTIES 1. Conduct comprehensive psychosocial, housing, and needs assessments to determine housing barriers, strengths, risks, and support needs. 3. Performs all procedures accurately, completely and efficiently for clients in accordance with applicable program policies and procedures. 4. Develop individualized, person-centered Housing Support Plans in collaboration with participants and interdisciplinary care teams. 5. Assist individuals in locating, securing, and maintaining permanent housing through housing search activities and landlord engagement. 6. Complete housing applications, affordable housing paperwork, subsidy applications, and supportive housing referrals. 7. Educate participants on tenant rights and responsibilities, lease requirements, and successful tenancy skills. 8. Coordinate move-in activities, including obtaining identification documents, household items, furniture resources, and utility assistance. 9. Provide ongoing tenancy sustaining services to prevent eviction and promote long-term housing stability. 10. Conduct home visits and community-based visits in accordance with Medicaid program requirements. 11. Monitor participant progress toward individualized goals and update service plans as required. 12. Coordinate care with primary care providers, behavioral health providers, substance use treatment programs, hospitals, managed care organizations, and community agencies. 13. Advocate for participants to access medical care, behavioral health services, benefits, transportation, employment services, and other community resources. 14. Provide crisis intervention and de-escalation during housing or behavioral health emergencies. 15. Identify early warning signs that may place housing stability at risk and develop intervention strategies. 16. Maintain regular communication with landlords, property managers, housing authorities, and supportive housing providers to address tenancy concerns. 17. Facilitate referrals to financial assistance, food resources, legal services, vocational services, and social support programs. 18. Assist participants in developing budgeting, money management, and independent living skills. 19. Maintain accurate, timely, and complete documentation in the electronic health record (EHR) and/or Medicaid documentation systems. 20. Ensure all documentation meets Medicaid billing standards, agency policies, and regulatory requirements. 21. Complete progress notes, service plans, assessments, authorizations, and encounter documentation within required timeframes. 22. Maintain participant confidentiality in accordance with HIPAA and agency policies. 23. Monitor participant eligibility and authorization periods for Medicaid Housing Support services. 24. Track performance measures related to housing retention, engagement, service utilization, and quality outcomes. 25. Maintain productivity standards and monthly service delivery expectations. 26. Participate in quality improvement initiatives and program audits. 27. Stay current on Medicaid regulations, housing resources, fair housing laws, and community services. 28. Perform other duties as assigned. QUALIFICATIONS EDUCATION: * Associate’s/bachelor’s degree in human services field masters preferred. * 1 year of experience working in case management or residential setting providing housing assistance to homeless clients. EXPERIENCE: * Strong understanding of trauma-informed care and person-centered planning. * Knowledge of motivational interviewing and harm reduction principles. * Excellent crisis intervention and conflict resolution skills. * Ability to establish effective working relationships with landlords, healthcare providers, and community partners. * Strong organizational, documentation, and time-management skills. * Ability to manage a diverse caseload while meeting productivity and compliance standards. * Effective written and verbal communication skills. * Knowledge of HIPAA, Medicaid regulations, and housing confidentiality requirements. * Proficiency with Microsoft Office and electronic health record systems. * Knowledge of Housing and Homelessness systems such as Coordinated Entry process, HMIS and HUD/ COC regulations. * Knowledge of the Housing first Model, diversion and prevention strategies and length of stay management
