Description HUMAN TRAFFICKING CASE MANAGER Lake County Task Force – A Safe Place Libertyville, Illinois Crisis Response | Survivor Advocacy | Intensive Case Management On-Call 24/7 Rotational Position About the Role The Human Trafficking Case Manager serves as a frontline responder and long-term advocate for survivors of human trafficking across Lake County. This position operates within the Lake County Task Force and collaborates directly with law enforcement, prosecutors, and community partners to ensure survivors receive immediate crisis support and comprehensive, trauma-informed services. This is not a desk job. This is field-based, relationship-driven work that requires emotional intelligence, crisis readiness, and strong system navigation skills. You will respond to law enforcement calls, conduct safety and danger assessments, develop individualized service plans, and walk alongside survivors as they rebuild stability, independence, and safety. This role reports to the Director of Human Trafficking and works collaboratively across agency programs. Core Responsibilities Crisis Response & On-Call Support Participate in 24-hour on-call rotation, including one weekend every 6–8 weeks (with two weekdays off) Respond to referrals from Homeland Security, FBI, Lake County Sheriff, and Lake County State’s Attorney’s Office Provide immediate onsite or offsite crisis intervention Conduct danger assessments and safety planning at intake and throughout services Intensive Case Management Conduct trauma-informed intakes and comprehensive needs assessments Develop and monitor individualized service and safety plans Provide ongoing advocacy, documentation, and follow-up across systems Assist with crime victim compensation claims and related documentation Ensure timely, accurate documentation in accordance with agency and funding requirements Survivor Support Services Provide intake and supportive services in areas including: Shelter and housing placement Public benefits enrollment (TANF, SNAP, Medicaid, childcare assistance) Employment readiness and workforce preparation Education access and documentation assistance Family support and reunification Legal advocacy and court accompaniment Medical and mental health coordination Substance use treatment referrals Transportation and basic needs support Community & Task Force Engagement Attend Lake County Human Trafficking Task Force meetings, Steering Committee, Coalition meetings, and Victim Identification Subcommittee Foster strong partnerships with law enforcement, legal partners, healthcare providers, and community agencies Participate in staff meetings, case consultations, and required trainings Maintain confidentiality consistent with agency and survivor safety standards What Success Looks Like Survivors feel believed, supported, and empowered Service plans are actionable and measurable Documentation is thorough and compliant Law enforcement and community partners view you as reliable and collaborative Crisis-situations are handled calmly, ethically, and strategically Requirements Qualification Minimum Requirements: Bachelor’s degree in Social Work, Counseling, Education, or a closely related human services field Experience in crisis intervention, case management, or victim advocacy Ability to work flexible hours, including evenings, weekends, and on-call rotation Strong written documentation and communication skills Valid driver’s license and reliable transportation Preferred: Bilingual (Spanish/English strongly preferred) Experience working with survivors of trafficking, domestic violence, or trauma Familiarity with public benefits systems and victim compensation processes Competencies: We are looking for someone who demonstrates: Trauma-informed care and survivor-centered advocacy Strong crisis response judgment Emotional regulation in high-stress situations System navigation and resource coordination skills Cultural humility and respect for diverse survivor experiences Ability to motivate clients toward stability and independence Professional boundaries and confidentiality integrity Compensation Wage starting at $25 per hour (26 pay periods/ paid bi-weekly) Benefits Benefits include health, dental, and vision insurance; a Flexible Spending Account (FSA); a 401(k) retirement plan; generous paid time off, including vacation, sick leave, and holidays; and access to an Employee Assistance Program (EAP).
