The CM Extender responsibilities focus on assisting the care team to support the member’s health care needs and to facilitate and organize appropriate member education, resources and alignment of care. This role provides direct support to the member through a collaborative role with the Innovations Care Manager or I/DD Care Manager Care Management Extenders complete supportive activities, inclusive of in-person support for member when indicated. Travel is an essential function of this position.    Role and Responsibilities:   Duties of the I/DD Care Management Extender include, but are not limited to, the following:   General Outreach and Engagement / Service Monitoring  * Assist with scheduling and coordinating appointments (e.g., assistance with scheduling appointments, providing wellness reminders, assistance arranging transportation, confirming participation in scheduled appointments, etc) * Provide outreach to members to provide information about a community resource or support * Engage members in health promotion activities and knowledge sharing * Assist with identifying residential vacancies which may meet needs of member * Submit and track referrals in NCCARE360 database to connect members to community service provider * Assist the care manager in monitoring consistency of service delivery, as triggered by threshold reports or requests from care managers * Review service documentation to monitor progress toward individualized goals and fulfillment of the intent of the service authorized * Provide back-up in-person monitoring support, as needed, for members in the NC Innovations waiver Benefits Consultation and Support to Members  * Ensure members know what benefits they are eligible to receive, with assistance to enroll in benefit plans as needed * Support member in navigating the DSS systems including but not limited to Medicaid reviews and applications, continued eligibility requirements, disability benefits through the Social Security Administration * Communicate with Medicaid and Medicare benefit program Case Managers to resolve issues * Assist with Medicaid C enrollment reconciliation and working with DSS to address IN indicator issues * Notify DSS of benefit issues and develop action plan to resolve Intensive Support (telephonic and in-person)   * Provide more intense telephonic or in-person support to members identified by care manager as needing support to address unmet health-related resource needs or other community resource needs.  This assistance includes, but is not limited to, the following:  * Assist with understanding processes and provide needed support related to guardianship, establishing alternatives to guardianship, restoration of rights * Provide education about appropriate rights and how to request accommodations related to housing, employment, education, etc * Assist with the development of life related emergency plans * Provide in-person support to member with completion of applications for housing, food, transportation vouchers, childcare assistance programs in the communities where the member lives and works, and monitors successful linkage to resources * Assist with housing needs (e.g., Identifying options for renting or purchasing a personal residence, assisting with purchasing furnishings for the personal residence, developing a housing support crisis plan that includes prevention/early intervention services when housing is jeopardized, etc) Collaboration with the Care Team and Provider Networks  * Attend huddles and meetings related to care planning and provider engagement, as needed * Notify Care Manager of any new service needs identified during contacts / service monitoring * Share information with the care manager and other members of the care team on the member’s circumstances (e.g. alerting care team members of an unexpected move, need or other event)   Knowledge, Skills, and Abilities:   * Basic knowledge of the Medicaid service system * Working knowledge of the evaluation, care and treatment of people with intellectual & developmental disabilities, mental health and co-occurring disorders. * Knowledge of community resources * Good organizational skills and ability to handle multiple priorities and deadlines * Must be able to prioritize and cope with stress in a positive manner * Ability to communicate effectively both verbally and in writing with member and care team * Proficient in Microsoft Office and web-based applications * Solid verbal and written communication skills * Ability to maintain confidentiality * Ability to work effectively and cooperatively with people from diverse backgrounds Demonstrates flexibility in organizational needs to perform other duties as assigned * Ability to negotiate and resolve conflict    Education/Experience Required:   * High school diploma or equivalent  AND   * Meets one of the following requirements: * A person with lived experience with an I/DD or a TBI with demonstrated knowledge of and direct personal experience navigating the North Carolina Medicaid delivery system * A parent or guardian of an individual with an I/DD or a TBI and has at least two years of direct experience providing care for and navigating the Medicaid delivery system on behalf of that individual * Has 2 years of paid experience performing the types of functions described and with at least one year of paid experience working directly with the LTSS population AND  * Must reside in North Carolina * Must have ability to travel regularly as needed to perform job duties Education/Experience Preferred:  Experience working with individuals with co-occurring physical health and/or behavioral health needs preferred.   Licensure/Certification Requirements:  N/A
