Job Title: IDT CNA Lead                                         FLSA Status: Non-Exempt Salary Range: TBD                                                      JOB SUMMARY: SERVES AS A MEMBER OF THE INTERDISCIPLINARY TEAM (IDT). SERVES AS TRANSPORTATION REPRESENTATIVE DURING IDT MEETINGS AS NEEDED. PERFORMS PATIENT CARE TASKS IN THE COMMUNITY AND AT THE ADULT DAY HEALTH CENTER. REPORTS DIRECTLY TO THE CENTER MANAGER.  SPECIFICATIONS Education: High school diploma or equivalent. Currently listed as NA1 or NA2 with NC Division of Facility Service with no substantiated findings of neglect, abuse, or misappropriation of resident’s property in a nursing home. Experience:1-2 years’ experience as a nursing tech/nursing aide, or EMT or corpsman. At least 1 years’ experience working with the frail elderly population. Number and Type of Employees Supervised (optional): None Licensure, Registry or Certification Required: Valid NC driver's license and vehicle.  Certificate of completion from a nursing tech/aide program. Special Training: Good driving record; able to communicate effectively both verbally and in written form; able to lift and turn participants; knowledge of and/or care of adults with chronic health care conditions.  Only act within the scope of his or her authority to practice.  Meet a standardized set of competencies for the specific position description established by Senior Total Life Care, Inc. and approved by CMS before working independently. Immunizations: Be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact Ages of Patients Rendered Care:   Adult    Geriatric   Key Responsibilities: (*denotes an age-related skill or task) ·         Provides personal care to participants of Senior TLC (bathing, dressing, feeding, toileting, transfers, lifting and incontinent care) in the Adult Day Health Center or in the Home as determined by the IDT. ·         Reports changes to the members of the IDT as appropriate.  ·         Participates as a member of the Interdisciplinary Team.  Attends daily IDT meetings, Service Delivery Request (SDR) Meetings, participant care planning and other meetings as needed.  ·         Takes at least monthly vital signs and weights for Senior TLC participants. ·         Supervises NuStep utilization in the day center as indicated by the therapy staff. ·         Serves as a liaison between the IDT and the Transportation Department communicating transportation needs of participants. Completes transportation reassessments and all initial CNA assessments and reassessments for participant care planning and service delivery requests, as needed. ·         Transports participants to day center and specialist appointments as determined by the IDT. ·         Scribes for the IDT team during IDT meetings and care planning. ·         Documents services provided and observations in the electronic medical record and/or electronic health record. ·         Participates in the CNA on-call process. ·         Is flexible in hours worked to accommodate the needs of the participants coming to the Adult Day Health Center. ·         Supports Senior TLC’s mission to encourage and support the quality of life of seniors wishing to continue living in the community; its vision to be the preferred provider of individualized care for seniors in the community; and its values of respect, integrity, accountability, compatible goals, and compassionate care. ·         Other duties as assigned.
