About This Role: The IHS Designated Coordinator is responsible for meeting with, setting up, and providing ongoing training and support to individuals accessing Accra’s Individualized Home Supports with Training (IHS) program. As an IHS Designated Coordinator, you will have the opportunity to: Assist participants in a 1:1 in-home and in-community setting providing support and skill development to maintain independence and teach individual life-skills. You may provide skill development in areas including but not limited to: community participation, health, safety, and wellness, household management, and adaptive skills. A day in the life as an IHS Designated Coordinator could consist of assisting a participant with the following: attending medical appointments, finding and accessing community resources, grocery shopping, budgeting and financial management (processing bills, balancing check books, etc.), communicating with the participants team members (case managers, guardians, etc.), and updating necessary paperwork. **This position requires travel to clients' homes in the surrounding Metro counties** Mileage reimbursement is included. Requirements Must meet one of the following requirements: Bachelor degree in a field related to Human Services, Education, or Health and one year of full-time work experience providing Direct Care services to persons with disabilities or persons age 65 and older, or equivalent work experience providing care or education to vulnerable adults or children. Associate degree in a field related to Human Services, Education, or Health and two years of full-time work experience providing Direct Care services to persons with disabilities or persons age 65 and older, or equivalent work experience providing care or education to vulnerable adults or children. Diploma in a field related to Human Services, Education, or Health from an accredited postsecondary institution and three years of full-time work experience providing Direct Care services to persons with disabilities or persons age 65 and older or equivalent work experience providing care or education to vulnerable adults or children. Minimum of 50 hours of education and training related to Human Services and disabilities; and four years of full-time work experience providing Direct Care services to persons with disabilities or persons age 65 and older under the supervision of a staff person who meets the qualifications identified in clauses (1) to (3). Additional requirements: 1 year experience in a Human Service field Demonstrated creativity, critical thinking, and problem-solving skills Excellent organizational, problem solving, communication, and interpersonal skills. Must pass a background study with the Minnesota Department of Human Services. Have a valid driver’s license, clean driving record and reliable transportation to use for work driving purposes. Ability to travel independently across the Accra geographic service areas.
