Are you a forward-thinking professional with a passion for improving health systems? We are seeking an Illawarra Shoalhaven HealthPathways Program Lead to join the dedicated team at COORDINARE.
In this core leadership role, you will be responsible for the efficient planning, delivery, monitoring, and evaluation of the HealthPathways program within the Illawarra Shoalhaven region. While your primary focus will be on the local site, you will also play a key role within the broader COORDINARE HealthPathways team, contributing to the program’s implementation and continuous improvement across South Eastern NSW and into the ACT.
A critical part of your role will be ensuring that HealthPathways content remains accurate, up to date, and locally relevant, enabling clinicians to make informed decisions at the point of care.
You will work closely with internal teams, supporting the Southern NSW & ACT HealthPathways program, and a wide range of stakeholders to embed HealthPathways into the daily workflows of GPs, hospital clinicians, nurses, and allied health professionals.
Your ability to see the role of HealthPathways in supporting person-centred care, build strong relationships, and lead meaningful change will be essential to strengthening the program and improving health outcomes across the region.
Join our dynamic team and thrive in a workplace that truly cares about your well-being and success:
