Silver Leaf In-Home Care is a respected, locally owned and operated care agency, that has been providing our seniors with quality, personalized care for over 27 years.
Currently, Silver Leaf is seeking a non-medical care manager to oversee the care services for its clients and scheduling of caregivers.
This position involves visiting clients’ homes to conduct assessments and create care plans, monitoring their health issues, ensuring their needs are being met and maintaining communication with family members.
Responsibilities also include matching caregivers with the necessary qualifications to meet the specific needs of each client, maintaining flexibility with occurring schedule changes and effectively managing hours worked using an on-line timekeeping system.
