JOB DESCRIPTION PRN RN/LVNs. We are looking for compassionate nurses with a passion for service, family values, and a true desire to help the elderly. RESPONSIBILITES Travel to assigned patient’s homes and manage their care plan according to physician’s instructions. Assess and chart observations of the patient’s condition at each visit. Complete evaluation tasks, including reviewing medication and vital signs. Dress or redress wounds and assess the healing progress. Educating caregivers and family on the aftercare or ongoing care of the patient. Keep the patient comfortable through palliative care. Recommend tools or devices that may improve the quality of life for the patient. Requirements SKILLS Strong interpersonal skills with the ability to build rapport, establish trust with patients, and deliver high-quality care that supports patient comfort and a positive care experience. . Able to take the initiative to make independent decisions when required and accept direction and requests from patients. The ability to follow instructions but also act independently if necessary. Excellent observational and problem-solving skills Ability to act swiftly in emergencies and work efficiently. Flexibility and patience to work in a variety of living environments. Knowledge of clinical skills including changing bandages, giving tube feedings, administering shots, and setting up and monitoring IVs REQUIREMENTS AND QUALIFICATIONS Candidates must have a minimum of 1 year of nursing experience working in the home care industry (or related industry – hospice, home health, hospital, etc.) Relevant license and certification that is current A valid driver’s license and reliable transportation RN pay rate: $45/hr LVN pay rate: $40/hr E-verify company EOE #FTHCA
