Living Through My Dreams Home Care, LLC is seeking dedicated, professional and qualified individuals to meet the ever growing need to service those in need of In-Home Assistants from adults ranging from ages 18, geriatrics and the home health population. We are seeking individuals to help build relationships with patients and their families and help to help our patients reach their maximum level of independence and comfort.

• Maintain a safe and hazard-free work environment by following policies and procedures of Living Through My Dreams Home Care, LLC
• Certified Nursing Assistance (CNA), or Medical Assistant (MA) in good standing with the State.
• Must be able to relate positively and favorably to facility staff, patients/residents, and families while working cooperative with others.
• Must meet all local health regulations and pass post-employment physical exam if required. This requirement also includes criminal background investigation and reference inquiry.
• Must maintain regular attendance on the job, including traveling daily to partner facilities.
• Must be able to perform the Essential Job Functions of this job, with or without reasonable accommodation.
• Prior experience in a Skilled Nursing Facility (SNF).
• Assist with daily living activities
• Help patients with personal hygiene
• Obtain and record vital signs and weight, nail care, foot care, skincare, dressing, feeding, toileting and elimination, and transferring patient
• Complies with Agency policies, procedures, and reporting requirements
• Identifies and communicates abnormal findings to the Supervisory Nurse
• The IHA must be able to fulfill all responsibilities and duties of the job description as posted
  Caseloads are built according to desired areas and times of availability. We are looking to hire team members who are looking for part-time, and contractual schedules. You can customize your caseload to meet your geographic and scheduling preferences.
