Responsibilities:
Implements and maintains a professional and caring relationship with all clients; providing reliable care, limit setting and boundaries while demonstrating understanding of the clients’ needs at all times.
Plans and participates in activities with the clients as outlined in the care plan. Attends and actively participates in weekly team meetings, group supervision, and completes in-service hours annually as required by state regulations.
Maintains a professional, courteous and helpful relationship with clients family and other primary social supporters.
Adheres to Stay Graceful’s Standard Operations Procedures and South Dakota State Regulations/Administrative Rules relating to in-home care.
Assures all clients, coworkers and family members are treated equally and with respect. Responsible for the quality of services and completion of necessary in-home care duties and related training including:
Medication Management
