Description

This is a professional position under the direction of an administrator for SFR program, in conjunction with the Health Services Supervisor. The Incident Manager is responsible for/or assisting in the reporting, follow up and facilitating reporting and oversight/collection of administrative review and follow up for all reportable incidents as outlined in Incident Management Policy. Additionally, the Incident Manager will complete reports, track incidents internal and reportable, as directed by administrator or appropriate designee.

  • Follow standardized incident reporting requirements including electronic, verbal and/or fax within the required time frames and to the appropriate agencies as per Merakey AVS Incident Management and Critical Event Reporting Policy.
  • Follow the Service Line Agreement with QCO to complete the EIM process, including notifying appropriate parties, requesting investigators and completing required sections within EIM.
  • Insure timely notification of family or legal guardian regarding reported incidents.
  • In the event a reportable incident affects the physical or emotional well being of an individual, insure prompt medical treatment or counseling/comfort measures are provided, when necessary.
  • Communicate findings to Senior Director or designee regarding the status of incidents.
  • Participate in meetings related to incident management and closing of incidents.
  • Insure incidents are finalized by due date.
  • Insure that all follow up action is completed as recommended and in a timely manner.
  • Gather information related to Department of Health surveys as directed by Senior Director or designee.
  • Communicate directly with Senior Director or designee to confirm Merakey/AVS standards of reporting are maintained and proper incident management procedures are followed at all times.
  • Maintain a tracking form of all EIM reports and internal incident reports submitted within the program.
  • Conduct investigations, where applicable, for internal, non-reportable incidents.
  • Participate in related committees as requested.
  • Promote the health, safety, rights and dignity of individuals receiving services.
  • Is flexible in regard to their work schedule.
  • Performs related work as required.
