PERSONALISATION PRACTITIONER

We are looking for practitioners who can engage creatively with people with disabilities on a 1:1 basis, supporting them to identify opportunities and activities that reflect their interests, promote independence, and lead to meaningful outcomes. QUALIFICATIONS AND SKILLS

  • Previous experience working with people with disabilities
  • Excellent communication skills and the ability to build a rapport
  • Good basic education
  • Please note: you will be required to become registered with SSSC and undertake relevant qualifications to enable you in your role
  • Highly Desirable: Driving license – with access to your own car
  • Personal qualities willing to work flexibly, a belief in inclusion

    Job Type: Full-time, Part-time with hours ranging from 20-30 per week, this will include weekends
    Salary: £12.60-£12.75 per hour
    Contract length: we are looking for commitment for a minimum of 12 months

  • Build relationships with the individual/s and their families to support them and their son/daughter
  • to participate in the planning, preparation and delivery of person centred/outcome focused leisure activities in a variety of settings or on occasion, in the individuals’ home.
  • To brief and de-brief parents/carers before and after events and to complete all necessary forms or reports.
  • to work collaboratively with staff at PLUS and from other organisations.
  • Implement the individual’s plan to achieve stated outcomes
  • Support young people with personal care such as toileting and feeding
  • Promote independence
  • Complete paperwork promptly (mostly electronic)
