Description ****Average hourly rate is $18.45 (including wage, incentives, bonuses, overtime, shift differential, etc.) **** We are looking for a Full-Time Independent Living Support Worker to work Monday-Friday 9a-5p. Pathway Family Services, LLC. provides resources for youth and families through their Independent Living Program, TRAIL (Teens Reaching Adult Independent Living), Psychiatric Residential Treatment Facility, Qualified Residential Treatment Program (QRTP), outpatient services and specialized foster care services. As an Independent Living Support Worker at TRAIL, you will promote the well-being of youth residing in the Teens Reaching Adulthood and Independent Living (TRAIL) program, participates in staff meetings and trainings and coordinates with staff from other agencies involved in providing services. Position provides direct supervision of youth and teaches independent living skills to young adults in the program. Be a Life Coach is who is responsible for showing apartments for STRIVE and DCF youth. Also responsible for collecting rent, putting in support tickets for maintenance on apartments, and ensuring background screening process is completed. In addition, you may be responsible for: Perform basic clerical and janitorial duties. Maintain neatness of the office area Auditing client files according to Quality Improvement standards Work flexible hours and additional shifts when required Provide a safe learning environment for adolescents in placement Provide information to the contract agency on all matters related to the adolescent, as required Appropriate modeling of life skills to youth in the program. Modeled skills include but are not limited to: assertiveness, communication, conflict management, problem solving and decision making Complete all paperwork, logs, critical incidents and monthly reports are required. Teach adolescents through individual and group process about basic independent living skills associated with shopping, housekeeping, budgeting, job skills, personal hygiene, health, peer interactions, anger management, time management, educational planning and cooking Ensures adolescents have the necessary household items to reside in the apartment Provides on-call support as necessary to youth and other staff. Provides non-physical alternatives for de-escalating and managing youth Provide educational and situational counseling (group and individual) for adolescents Monitors youth participation with community resources for the benefit of youth’s employment, education, mentorship and volunteerism Completes documentation and forwards reports containing descriptive, analytical and evaluative content Shall inspect youth’s apartment as needed to ensure the safety and security of youth Will be required to drive personal or agency vehicle while transporting children or conducting agency business Refrains from assuming any duty that is unrelated to and/or interferes with the responsibilities of the position Advocates, promotes and practices cultural sensitivity and responsiveness in all day-to-day interactions Develops, promotes and practices teamwork in all activities. Works collaboratively with the Independent Living Specialist, youth and referring workers to ensure compliance with each youth’s individualized written plans. Qualifications: High School Diploma/G.E.D. is required and at must be least 25 years of age is required per stated regulations that employees must be at least 3 years older than the oldest resident. Must have a valid driver’s license and meet agency underwriting standards.
