ABOUT THE ROLE

Care and Love Pty Ltd is seeking an experienced and compassionate NDIS Independent Support Coordinator to join our team. This role is ideal for professionals who are passionate about empowering people with disabilities to live independently and reach their goals through the National Disability Insurance Scheme (NDIS).
You will be the key liaison for clients, families, and service providers, ensuring that supports are effectively coordinated, aligned with participants’ needs, and responsive to their evolving goals.

KEY RESPONSIBILITIES

• Develop and manage individualised NDIS support plans in partnership with clients and their families/carers
• Coordinate and monitor service delivery across multiple providers
• Build and maintain strong relationships with clients, providers, and stakeholders
• Assist clients in understanding and navigating the NDIS system
• Regularly review plans and progress, making updates as needed
• Maintain accurate and compliant records in line with NDIS and organisational requirements
