The Institute for Urban Indigenous Health Ltd was established by its founding members to provide a coordinated/integrated approach to the planning, development and delivery of comprehensive primary health care services for Aboriginal and Torres Strait Islander populations within the Southeast Qld Region. The Regional and Specialist team includes Integrated Specialist Services, Mental Health and Wellbeing Services, Primary Healthcare Services, Birthing and Early Childhood Services (South), and Child and Youth Services. Our purpose is to connect, guide, and empower teams and communities through culturally safe, transparent, and responsive ways. About the Role? The Indigenous Outreach Worker supports effective improved access for Aboriginal and Torres Strait Islander people to available health care services in the designated region. This includes effective liaison with local Indigenous Communities/groups, identification of barriers to access, implementation of practical solutions and provide practical assistance to identified Aboriginal and Torres Strait Islander peoples to manage primary health care related needs. Your day to day duties may include: Establish strong cultural ties and links with local Aboriginal and Torres Strait Islander communities to encourage and support the increased use of health services, including Aboriginal and Torres Strait Islander Health Checks, and to identify Aboriginal and Torres Strait Islander people who would benefit from improved access to these health services. Develop and maintain productive working relationships and alliances with identified GP Clinic staff, relevant service providers, businesses and community organisations. Assist clients with attending GP consultations, including first and follow up appointments. Assist clients with non-health specific needs (such as access to Centrelink, employment, other psycho-social needs) – and record this assistance on MMEx About You?
