La personne est responsable de fournir des services professionnels de promotion de la santé dans la communauté. Elle fournit des interventions et des traitements dans la clinique ou dans la communauté en collaboration avec les clients pour assurer la continuité de leurs soins.

DESCRIPTION

Centre de santé Saint-Boniface is a community health centre that provides bilingual services to Winnipeg’s French-speaking population and St. Boniface and St. Vital residents. Programs and services at the Centre are provided by an interdisciplinary team.
The incumbent is responsible for providing professional health promotion services in the community. The incumbent provides interventions and treatments in the clinic or in the community in conjunction with clients to ensure continuity of care.

JOB REQUIREMENTS

  • Bachelor of Science in Nursing recognized by the province of Manitoba.
  • Certificate of Practice issued by the College of Registered Nurses of Manitoba.
  • Active registration with the College of Registered Nurses of Manitoba.
  • Three to five years of experience in a community setting, public health or health education and promotion.
  • Extensive knowledge of health promotion, development concepts and community resources.
  • Be able to meet the needs of a vulnerable population.
  • Must be able to do moderate lifting, sit for long periods of time, and walk during the day.
  • Be physically able to perform the duties of the position in a variety of weather or environmental conditions.
  • Have very good oral and written command of both of Canada’s official languages.
  • Demonstrate critical, innovative and proactive thinking to meet the needs of clients and coworkers.
  • Possess excellent communication, conflict resolution and interpersonal skills.
  • Maintain strong internal and external working relationships.
  • Manage time effectively and be able to set priorities and multi-task in a fast-paced environment.
  • Proficiency in Microsoft Office (Outlook and Word) (knowledge of Excel, TEAMS and SharePoint would be an asset).
  • Hold a valid Basic Life Support Certificate.

Responsibilities
  • Provide professional, integrated primary care services focusing on health promotion and disease and injury prevention.
  • Consult with the interdisciplinary team to evaluate and manage the client’s file in conjunction with the client or family.
  • Document clients’ clinical care as promptly as possible.
  • Support the implementation of care plans in conjunction with the interdisciplinary team to manage the complex health needs of clients.
  • Consult with physicians, nurse practitioners and other members of the interdisciplinary team with respect to various client- related situations.
  • Sort and respond to telephone, in person or email information requests; determine how urgent request are, and follow up as required.
  • Maintain ties with community groups and other health service organizations by participating in community meetings and events.
  • Provide customer service in accordance with the Registered Nurses Act and Acts governing the College of Registered Nurses of Manitoba.
  • Contribute to the annual planning, implementation and evaluation of the Centre’s initiatives and participate in community initiatives as required.
  • Coordinate internships for nursing students in conjunction with the Health Promotion and Education Nurse and ensure they run smoothly.
  • Critically review research reports and health promotion literature.
  • Adopt an evidence-based practice with people and communities.
  • Participate and contribute to group education sessions or workshops relating to his/her department.
  • Participate in research activities, as appropriate.
  • Meet with supervisor regularly to review files.
  • Demonstrate diligence, confidentiality and loyalty in dealings with employer.
  • Ensure effective communication within his/her department and between departments.
  • Work with managers during the development and implementation of the operational plan and the evaluation of initiatives, particularly within his/her department.
  • Know and comply with all organizational policies, procedures and standards, as well as legislative, regulatory, professional and accreditation requirements.
  • Keep up to date on appropriate handling and transportation of hazardous products; comply with the standards outlined in the Infection Control Manual, Emergency Preparedness Manual and the Workplace Hazardous Materials Information System (WHMIS).
  • Perform other tasks and duties related to this job description that do not exceed the skills and abilities described.
