Job Title: Information & Referral Coordinator Department: Administration Hourly/Non-Exempt Reports To: Executive Director Work Schedule: Full-time/Monday-Friday; 8:00am-4:30pm Position Summary I&R Coordinator assists seniors through active listening, effectively identifying and connecting people in need to available community resources, while demonstrating respect and compassion. I&R Coordinator serves as the first point of contact for calls and visitors in need of assistance and may also be involved in follow-up communications, intake for specialized programs or services, outbound calls, and community outreach. Essential Functions Provide thorough, appropriate, and helpful referrals, and information, to address a caller’s needs(s) Responsible for assessing potential clients for program eligibility for home delivered/congregate meals. Establish new client files and enter information in database. Coordinate information and referral services. Responsible for yearly certification as OSHIIP County Coordinator, including knowledge of Medicare eligibility for the older adult population. Certification available. Assist clients in identifying appropriate and available resources, or in locating needed information. Training available. Assist clients with applications and forms. Assist clients with their efforts to access other service systems. Develop and maintain resource files. Maintains client confidentiality. Other duties as assigned by the Executive Director. Requirements Required Qualifications Applicants should possess motivation to help people, a non-judgmental attitude, excellent people skills, and an ability to empathize. High School diploma/GED required. Associate’s degree in a health and human services related field preferred OR one-year minimum experience in health and human service-related field is a plus. Proficiency in Microsoft Office Suite First-hand experience with office equipment (e.g., fax machines and printers) Professional attitude and appearance Solid written and verbal communication skills Ability to be resourceful and initiative-taking when issues arise Excellent organizational skills Multitasking and time-management skills, with the ability to prioritize tasks Knowledge of community resources or ability to learn. Ability to work with minimal supervision. Effectively prioritizes work and establishes clear goals and plans Must be able to pass pre-employment drug test and random alcohol and drug testing Must be free of disqualifying offenses per criminal background check
