Innovation Coordinator works under the guidance of the Regional Manager of Innovative Services with the focus of Coordinator is to support people to live more effectively and meaningful lives by supporting them to secure nontraditional services that encompass a combination of staff supports and enabling technologies. This includes, residential, employment, and community supports. Key areas include, assessment, person centered / MAP’s planning, linking people to services, training, and advocacy. We are looking for Innovation Coordinator to help us develop and implement a unique and new service in Tennessee designed to help young adults in the communities where they live, learn, work, and play. Using a unique approach built around person centered, employment first, and technology first thinking. We are looking for Innovation Coordinator grounded in person centered thinking, who can foster a creative and forward-thinking approach to providing services in Tennessee.
The Charles Lea Center is a CQL accredited organization that is based in Spartanburg South Carolina and provides services in South Carolina, and North Carolina and recently was awarded a contract with the Tennessee Department Disability and Aging (DDA)to provide services in their State. QUALIFICATIONS:
REQUIREMENTS:
Bachelor’s Degree in Human Services or a related field with two (2) years of experience in providing or coordinating services to people with disabilities and special needs and/or with case management services.
-OR-
Four (4) Years Case Management experience in providing or coordinating services to people with disabilities and special needs and/or with case management services.
