The role you’ll contribute: The Inpatient Care Coordination Specialist will be dedicated to superior customer service and satisfaction. This position will develop and maintain positive relationships with both internal and external customers while conveying a positive image. Coordinates and directs patient care to ensure patients' needs are met and hospital policy is followed. The value you’ll bring to the team: Schedules follow-up appointments with physicians for patients discharged home. Coordinates appointments for patients without a documented primary care physician. Arranges specialist appointments as prescribed by the physician. Contributes to the daily processes involved in discharging patients. The expertise and experience you’ll need to succeed: 3+ years Work Experience Required
