Community Healthcare of Texas has provided Hospice and Palliative Care Services since 1996.  Community Healthcare of Texas has cared for patients with serious and terminal illnesses throughout North Central Texas.  Providing compassionate care for those living with an illness while supporting those caring for a loved one is the mission of Community Healthcare of Texas. Community Healthcare of Texas is currently recruiting for an Inpatient RN.  POSITION SUNNARY IPU RN/Case Manager assesses patient's physical, psychosocial, and spiritual condition and determines and/or revises the plan of care; provides palliative nursing care.   ESSENTIAL FUNCTIONS * Provide emotional support for patients and families/caregivers while instructing in end-of-life care/issues.  * Case manages a total plan of care for patient/support persons by assessing conditions, coordinating with team members, communicating with IDG, and supervising CNAs.    * Document nursing care and significant interactions with patients at the time of visit; support team members.    * Participate in follow-up with bereaved families with various types of contact.    * Document nursing care and significant interactions with patients at the time of visit; support team members.   * Discharge patients and coordinate transfer including instructing patients/caregivers in follow-up care.   * Manage multiple patients and their support persons simultaneously in crisis with acute symptoms including pharmacology.   * Manage assigned patients equally, regardless of severity, contagion, or complexity of diagnosis.    * Responsible for ensuring and assisting in providing a safe and clean environment for patients and families.  * Assist in housekeeping needs that are compliant with infection control procedures and policies. QUALIFICATIONS * Registered Nurse Licensed by the State of Texas * Must have emotional/spiritual maturity to deal with patients/support persons according to their needs in a crisis. * Be able to work autonomously within guidelines/protocol (without immediate support available) * Able to use a range of strategies to solve problems care/treatment modalities. * One year of nursing experience is required. * Hospice/Home Health experience preferred HOURS Working every other Saturday/Sunday night 7:00pm to 7:00am Reasonable Accommodations Statement  To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
