Description Be the first voice of care. As an Intake Assistant at Granite VNA, you play a vital role in connecting patients and families to the home health and hospice services they need—setting the tone for compassionate care from the very first call. Position Overview Reporting to the VP of Strategy, the Intake Assistant is responsible for responding to all inquiries and assisting with the patient intake process for Home Care and Hospice. Coordinates the referral process for new patients. Essential Functions Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. Accurately enters all information for new referrals from Concord Hospital into referral Manager and Electronic Medical Record (EMR) Accurately enters all information for resumptions of care ? Attaches referrals accurately to the Patient Medical Record Faxes Prehab referrals to appropriate agencies ? Enters verbal orders accurately and timely Enters pending referrals accurately and timely into Referral Manager Maintains a working knowledge of Granite VNA’s contractual relationships, insurance plans and managed care managers for initial authorization of services Manages and oversees the Granite VNA referral email group Daily reconciliation of Concord Hospital dischargers and transfers; includes adding the appropriate liaison and event code Other Job Functions Seeks out assistance of the Intake Nurse for support and clarification of unfamiliar areas Demonstrates flexibility in the role by accepting other duties as assigned for which qualified. Enters weekly time sheet according to agency policy Adheres to work schedule and attendance according to agency standard Adheres to Telecommunication and Personal Cell Phone Use guideline Understands and demonstrates compliance with agency policies and procedures Complies with infection control and safety measures in accordance with agency policy and procedure Completes all agency’s policies regarding client’s privacy, confidentiality and HIPAA requirements ? Adheres to the Agency’s standard of professional appearance and the Agency’s dress code policy Work Schedule: Friday, Saturday, Sunday & Monday 8:00 a.m. - 6:00 p.m. Requirements Associates degree or High School Diploma with equivalent work experience Ability to work in a fast-paced environment and multi-task Proficient with data entry, accurate and detail oriented Proficient in use of MS Office, including Word and Excel Excellent communication and customer service skills Familiarity with medical terminology
