Overview We are seeking a compassionate, capable, and caring Full-Time Intake Coordinator to join our team at our corporate office in Long Beach. The Intake Coordinator will play a key role in the conversion of new referrals, guiding clients through the onboarding process, and building strong, ongoing relationships with a variety of referral sources to ensure a smooth and supportive intake experience. Available: Monday-Friday This is not a remote position Responsibilities 1. Conversion of New referrals Achieve referral conversion according to department standards Timely outreach based on company guidelines Maintain up-to-date client status to share with referral sources Partner with referral sources to obtain updated client information 2. Effective Client Onboarding Educate clients on services for successful transition to care Document activities as directed Share information with staffing to have a successful caregiver match 3.
