HOME HEALTH INTAKE REFERRAL COORDINATOR Are you highly organized, detail-oriented, and passionate about helping patients begin their care journey with confidence? Join our growing home health team as an Intake Referral Coordinator, where you’ll play a vital role in ensuring patients receive timely, compassionate, and seamless access to care. In this fast-paced position, you’ll serve as the central point of communication between referral sources, patients, families, clinicians, and insurance providers—helping coordinate every step of the admission and intake process. WHAT YOU’LL DO As an Intake Referral Coordinator, you will: * Receive, review, and accurately document incoming referrals in a timely manner * Verify insurance eligibility, benefits, authorizations, and certifications * Coordinate with Nursing Supervisors and interdisciplinary team members to ensure smooth patient admissions and continuity of care * Maintain accurate patient records within the EMR system * Communicate professionally and effectively with referral sources, patients, families, and healthcare professionals * Respond to patient and agency needs with urgency, professionalism, and compassion * Support quality improvement initiatives and participate in the Agency’s QAPI program * Participate in ongoing training and professional development opportunities * Promote agency values, policies, and commitment to exceptional patient care WHAT WE’RE LOOKING FOR * Minimum of 1 year of referral intake experience preferred * Experience with Medicare, Medicaid, and third-party payers highly preferred * Medical office or healthcare background preferred * Knowledge of medical terminology * Strong customer service, communication, and organizational skills * Excellent computer and data-entry skills * Experience with HomeCare HomeBase (HCHB) preferred * LVN/LPN license preferred * Valid Driver’s License and current auto insurance required WHY JOIN US?
